We provide ongoing care and specialized life-saving treatment to patients, often following surgery and other serious procedures. Throughout the recovery journey, patients often require customized, intense therapies tailored to their needs as they work toward maximum health and regain function after stroke, brain injury, pneumonia, heart failure, COPD (chronic obstructive pulmonary disease) exacerbation, GI (gastrointestinal) obstructions, dehydration, acute kidney failure, uncontrolled diabetes, anemia/blood transfusion, or other serious infections.

Need for Higher Level Of Care
In cases where a higher level of care is required, we will stabilize your medical needs and coordinate your transfer to the closest hospital or hospital of your choice. Your provider at Mahnomen Health will communicate directly with the receiving physician and facility for a smooth transition of care.
Need for More Recovery Time
When extra time to recover and ongoing needs exist, we transition patients to the Care Center for your Medicare Part A Stay. Patients and their families appreciate the chance to access all the necessary rehabilitation services and medical care in one place while recovering close to home.
