We offer recovery care services to our patients who need some extra time to heal and strengthen after an acute care stay before returning home.
Recovery care services are sometimes referred to as ‘swing bed’ services; a transition or swing from an acute care stay to skilled nursing care in a hospital setting prior to returning home.
Swing bed care is often provided to patients recovering from:
Knee replacement
Hip replacement
Stroke or heart attack
Wounds needing wound care management
Patients requiring intravenous therapy
Promoting a Feeling of Wellness
Swing bed patients are encouraged to do as much for themselves as possible, wear their own comfortable clothing, and resume parts of their normal routine, while in a safe and accommodating setting. Patients are able to meet the challenges of daily living with renewed confidence and enthusiasm, while having medical support and rehabilitative therapies onsite. While our patients are receiving swing bed services, it is likely that the provider will see the patient less frequently than while they were in acute care. Our nursing staff will contact the provider for special or emergency needs at any time during a patient’s stay.
Mahnomen Health care team works with patients to set daily goals, make treatment decisions, and heighten communication around a workable discharge plan. Family members are an important part of this process and are welcome in the discussions and planning when patients request their involvement.
As a general rule, in order to qualify for these services, you must have had an acute care hospital stay for at least three days.
